The incision starts from the hairline, continues in a natural crease in front of the ear, inside the ear cartilage, and ends around the ear lobule behind the ear as needed. If a necklift is needed, a small incision may be made under the chin.

The underlying muscle fascia is then tightened and the fat elevated to appropriately correct for the anatomical changes of the aged face. The excess skin is then excised.
